“Content” = all intellectual property, proprietary information, trademarked and copyrighted material and all original works on our website, including, but not limited to words, documents, blog posts, data, information, design, website layout, artwork, graphics, color scheme, logos, proprietary information, branding, photographs, audio recordings and video recordings.

“Personal Information” = information we may collect, use, request, or that you may provide to us voluntarily, and which may be used to identify you. Personal Information may include, but is not limited to, your name, email address, phone number, address and health information.

“Protected Health Information” or “PHI” = individually identifiable health information as defined by HIPAA.

NO DOCTOR-PATIENT RELATIONSHIP

IMPORTANT: Use of this website does not create a doctor-patient relationship between you and Dr. Sarah Hollander or Psychiatry of Life LLC. A doctor-patient relationship is only established after completion of our intake process, execution of appropriate consent forms, and your first scheduled appointment.

TELEHEALTH SERVICES

If you elect to receive telehealth psychiatric services, you will be required to execute separate consent forms specific to telehealth treatment. All telehealth services are subject to additional terms and conditions outlined in our Telehealth Informed Consent and other clinical documents.

MEDICAL INFORMATION AND EMERGENCY PROCEDURES

A. No Medical Advice
The information on this website is not intended or implied to be a substitute for professional medical advice, diagnosis, or treatment. All content is for general information purposes only.
B. Emergency Services
Psychiatry of Life LLC does not provide 24/7 emergency psychiatric services. For psychiatric emergencies:

  • Call 988 (Suicide & Crisis Lifeline) for crisis support
  • Contact appropriate state crisis lines as provided in our clinical materials
C. State Licensing Limitations
Dr. Sarah Hollander is licensed to practice medicine in New Jersey, New York, and Michigan only. Services are only available to patients physically located in these states during telehealth sessions.

PAYMENT AND BILLING

A. Private Pay Practice

Psychiatry of Life LLC is a private pay practice. We do not submit claims directly to insurance companies but will provide documentation (superbills) for potential insurance reimbursement.

B. Fee Structure
Our current fee structure is available on our website and is subject to change with notice. Payment is due at the time of service.
C. Financial Responsibility
You acknowledge and agree that you are financially responsible for all charges incurred for services provided to you or authorized by you.
